Clinical notes
A complete note, in a few clicks.
Most clinicians finish notes at the end of the day. Vitae is built so the note is done when the visit is.

How a note comes together
Talk. Click. Done.
You review. You sign. Nothing is final until you say so.
- Your practice builds the templates. An administrator creates note templates, such as a progress note, from sections like HPI, Mental Status Exam, and Assessment & Plan.
- Each section has keyword menus. A keyword adds a saved sentence or opens a short list of options. Some lists allow one choice, others allow several.
- All Normal handles routine findings. Use it for one section or the whole note. It skips any section you have already written in.
- Finalize with AI writes the full note. Vitae sends your selections and text together and returns one complete note.
- You review, edit, and sign. Nothing is final until you sign.
Your templates, your way
- Change sentences, hide keywords, and add your own
- Reorder sections without changing the practice template
- Practice updates flow through, and your personal changes stay in place
- Reset to default returns any section to the practice version
Supervised providers
Notes that need a co-signature go to the supervising provider’s review list. The full history of each note is kept.
What the AI does, and what it doesn’t
The AI organizes, drafts, and structures the note from what you selected and wrote. It doesn’t sign, choose codes, or make clinical decisions. Coding stays with the clinician and the billing team.
Design goal
A stable follow-up visit, charted in under a minute.
That is the goal the note workflow is designed around.
Built by practicing clinicians, for the practices they know.
